- A — Accountable: Owns the outcome. Only one per task.
- C — Consulted: Provides input before the work is done (two-way).
- I — Informed: Notified when work is complete (one-way).
Common Mistakes to Avoid
- Multiple A's per task (accountability diffusion)
- No R assigned (nobody actually doing the work)
- Over-consulting — too many C roles slows every decision
- Treating I like C — informed parties don't give input
